Background

The Texas Tech University Health Sciences Center (TTUHSC), part of the Texas Tech University System, is seeking proposals for Implementation Services related to the Collaborative Care Model (CoCM). The goal is to enhance healthcare delivery across West Texas, which is underserved in terms of health professionals and facilities. TTUHSC aims to improve patient care through effective implementation and optimization of the CoCM.

Work Details

The Contractor will provide clinical, operational, and financial expertise to ensure model fidelity, sustainability, and high-quality patient outcomes. Key services include:

1) Clinical and Operational Technical Assistance

- Authoring workflow process maps for clinics. - Conducting quarterly operational audits. - Building a standardized CoCM Billing Toolkit and performing claims-denial reviews.

2) Patient Funnel Analysis

- Analyzing registry data to optimize Behavioral Health Care Manager (BHCM) panels.

3) Provider Engagement

- Hosting CME-accredited training sessions for primary care providers (PCPs).

4) Program Expansion Support

- Providing evidence-based tools for pediatric populations.

5) Training and Resource Delivery

- Customizing clinical templates and managing continuing education compliance.

6) Implementation Timeline

- The project will be executed in three phases over 24 months, focusing on infrastructure building, launch support, and long-term sustainability.

Period of Performance

The anticipated term of the agreement will begin upon execution and expire on August 31, 2028.

Place of Performance

Services will be performed across three distinct health systems within West Texas.

Bidder Requirements

Proposers must be current with the State of Texas Comptroller of Public Accounts and provide a Certificate of Franchise Tax status. They must demonstrate evidence of necessary certifications, permits, licenses, and insurances. Compliance with the Texas Risk and Authorization Management Program (TX-RAMP) is required. Proposers must also certify that they are not listed under certain sections of the National Defense Authorization Act or controlled by foreign adversaries.
