Eyeglass Lenses - DOC-LSP

Background

The Louisiana Department of Corrections is seeking to establish a Blanket Order Contract for the provision of eyeglass lenses. This contract aims to support the eye care needs of offenders at the Louisiana State Penitentiary, specifically through the RE Barrow Treatment Center Eye Clinic. The goal is to ensure that inmates receive necessary eyewear based on prescriptions provided by the agency's eye clinic.

Work Details

The contractor will provide a total of 4,000 eyeglass lenses over the contract period, which includes various specifications: 1. 1,200 single vision plastic lenses (plano to 4.00 sphere to 4.00 cyl), unfinished without scratch or UV coating. 2. 120 single vision plastic lenses (4.00 to 7.00 sphere to 4.00 cyl), unfinished without scratch or UV coating. 3. 40 single vision plastic lenses (7.00 to 12.00 sphere to 4.00 cyl), unfinished without scratch or UV coating. 4. 90 single vision plastic lenses (12.00 to 20.00 sphere to 4.00 cyl), unfinished without scratch or UV coating. 5. 2,720 bifocal lenses (ft28 bifocals, plano to 4.00 sphere to 4.00 cyl), unfinished without scratch or UV coating. 6. Additional quantities of various other lens specifications including polycarbonate and finished options are also required. All deliveries are expected within five business days from receipt of order.

Period of Performance

The initial contract period will begin on July 1, 2026, or upon date of award, whichever is later, and will end on June 30, 2027. The contract may be renewed for two additional twelve-month periods at the same prices, with a total contract duration not exceeding thirty-six months.

Place of Performance

The products will be delivered to the DOC LA State Penitentiary -911 Warehouse located at 17544 Tunica Trace - Hwy 66, Angola, LA 70712.

Bidder Requirements

Bidders must be registered in the LaGov system and comply with all applicable state laws and regulations regarding procurement and contracting in Louisiana.
