Background

The Commonwealth of Kentucky, through the Finance and Administration Cabinet, is issuing this solicitation for Chemical Analysis Testing Services on behalf of the Cabinet for Health and Family Services. The goal is to establish a Master Agreement for laboratory services that will support the Consultation and Compliance branches of Kentucky's Occupational Safety and Health Administration (OSHA). This contract aims to ensure that the necessary chemical analysis testing is conducted in compliance with occupational safety standards.

Work Details

The contractor will provide Chemical Analysis Testing Services, which include: - Analysis of organic solvents, inorganics, and physical analysis. - Samples may include air samples, bulks, and wipe samples submitted from both Consultation and Compliance branches. - The contractor must be accredited by AIHA, NIOSH, and IHPAT testing program. - Pricing must be provided for the first analyte and each additional analyte. - Results must be delivered within seven (7) business days. - No subcontractors are permitted for any analytes. - The contractor must furnish copies of chromatograms, calculations, and quality control documentation upon request.

Period of Performance

The initial contract period will be one (1) year from the date of award, with options to extend for four (4) additional two-year periods upon mutual agreement.

Bidder Requirements

Bidders must comply with specific requirements including: - Must be registered in the Commonwealth eProcurement System. - Must submit a Kentucky Tax Registration Application (Revenue Form 10A100). - Must provide a Certificate of Authority if a foreign entity. - Must comply with nondiscrimination clauses as per KRS 45A.560 to 45A.640. - Must disclose any violations of statutes within the last five years as per KRS 45A.485.
