Breath Alcohol Operator's License Envelopes

Background

The Commonwealth of Virginia's Department of Forensic Science (DFS) is a nationally accredited, independent forensic laboratory system established by Virginia law. DFS performs scientific analysis and examination of evidence in criminal matters for state and local law enforcement agencies, medical examiners, and Commonwealth’s Attorneys in Virginia. The goal of this contract is to furnish Breath Alcohol Operator’s License Envelopes to support the agency's operations.

Work Details

The contractor shall furnish and deliver Breath Alcohol Operator’s License Envelopes to the DFS Central Laboratory. The specifications for the envelope include: - Size: Approximately 2 3/8” x 3 9/16”. - Printing: The envelope must be printed on both the outside and inside as shown in Attachments A and B. - Design: The outer edge flaps of the middle pane shall be folded towards the inside, creating a pocket for a plastic identification card (approximately the size of a credit card or driver’s license). The inside printing must include a gum-bar area for temporary closure.

Period of Performance

The initial contract period will be one year with four optional one-year renewal periods.

Place of Performance

All work will be performed at the DFS Central Laboratory located at 8850 Times Dispatch Boulevard, Suite 100, Mechanicsville, VA 23116.

Bidder Requirements

Bidders must comply with all applicable federal, state, and local laws. They are required to submit bids electronically via eVA by the closing date and time. Additionally, bidders must provide bid samples upon request that represent all components specified in the scope of work.
