Background

The City of Philadelphia is seeking to provide supportive housing and case management services for 15-20 individuals transitioning from local incarceration. This initiative aims to assist these individuals in reintegrating into the community, thereby supporting the City’s mission of enhancing public safety and improving the quality of life for its residents.

Work Details

The contractor will be responsible for delivering supportive housing and case management services, which include: 1. Providing stable housing for 15-20 individuals transitioning from incarceration. 2. Offering case management services that include assessment, planning, coordination, and monitoring of services to ensure that the needs of the individuals are met effectively. 3. Facilitating access to additional resources such as employment training, mental health services, substance abuse treatment, and other necessary support systems. 4. Developing individualized service plans tailored to each participant's unique needs and circumstances. 5. Collaborating with local agencies and organizations to enhance service delivery and support networks.

Period of Performance

The contract will have an initial term not exceeding one year, with options for additional terms that may extend the total duration up to five years, contingent upon funding availability.

Place of Performance

Services will be performed within the City of Philadelphia, specifically targeting areas where individuals transitioning from incarceration reside or require assistance.

Bidder Requirements

Bidders must demonstrate compliance with applicable laws and regulations regarding service delivery. They must possess all necessary licenses and certifications required to operate in Pennsylvania. Additionally, bidders should not have any outstanding debts to the City of Philadelphia and must comply with non-discrimination policies as outlined in relevant city codes.
