Background

The Commonwealth of Kentucky's Cabinet for Health and Family Services (CHFS) is seeking a contractor to provide podiatry services for patients and residents at the Western State Hospital (WSH) and Western State Nursing Facility (WSNF). WSH is a 175-bed acute care psychiatric facility, while WSNF is a 144-bed skilled nursing facility located in Hopkinsville, Kentucky. The goal of this contract is to ensure quality podiatric care for individuals with various medical needs, including those with persistent mental illness and developmental disabilities.

Work Details

The contractor will provide on-site podiatry services for a maximum of three days per month, including one all-day clinic each month. Services include evaluating and treating conditions such as fungal nail infections, corns, calluses, ingrown toenails, and providing routine foot care for diabetic individuals. The contractor must maintain documentation of medical records and progress notes, work under the direction of the facility's Medical Director, and participate in training programs for staff as needed. Additionally, the contractor must provide their own medical equipment necessary for the services.

Period of Performance

The contract will commence upon execution and will continue as specified in the RFP until terminated or completed based on the needs of the facility.

Services will be performed at Western State Hospital and Western State Nursing Facility located in Hopkinsville, Kentucky.

Bidder Requirements

Bidders must be licensed podiatrists in Kentucky with at least three years of experience. They must also pass background checks, including criminal record checks and drug testing. Compliance with HIPAA regulations and other relevant licensing requirements is mandatory. Additionally, contractors must provide proof of CPR certification if required by their position.
