This is a SOURCES SOUGHT SYNOPSIS announcement to be used for preliminary market research purposes only. No proposals are being requested or accepted with this synopsis. This is not a Solicitation and no contract shall be awarded from this synopsis. No reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow up information requests. The purpose of this synopsis is to gain knowledge of interest, capabilities and qualifications of various members of Small Business, 8(a), HUBZone, Service-Disabled Veteran-Owned Small Business (SDVOSB), and Women-Owned Small Business (WOSB) to compete and perform service under a contract for the attached Performance Work Statement. A Request for Quote (RFQ) solicitation and firm fixed priced contract is contemplated depending upon the responses to this synopsis. This synopsis summarizes the requirements for the contractor to provide basic medical services at Great Onyx Job Corp Center (JCC) in Mammoth Cave, KY. Great Onyx JCC is located at: 3115 Ollie Ridge Rd.; Mammoth Cave, KY 42259. The contractor shall perform basic medical services at Pine Ridge JCC in accordance with the attached DRAFT Performance Work Statement. All responses shall be submitted electronically to the Contract Specialist, Louis Lieb at louis.lieb@usda.gov. At minimum responses should include a capabilities statement, and your SAM Unique Entity Identifier (UEI).

Background The Great Onyx Job Corps Center (JCC) is part of the USDA Forest Service Job Corps Civilian Conservation Centers (JCCCC), which provide academic, vocational, and life-learning skills to at-risk youth and young adults across the United States. The purpose of this contract is to provide basic medical services at the Great Onyx JCC located in Mammoth Cave, KY, ensuring compliance with applicable federal laws and guidelines. Work Details The contractor shall provide on-site medical services for all students at the Great Onyx JCC, including but not limited to: examinations and assessments, treatment, issuance of prescriptions and immunizations, monitoring health conditions, recommending hospitalization, follow-up care, and prevention planning. Services must adhere to federal regulations, OSHA and HIPAA requirements, as well as state and local laws. The contractor will coordinate medical services with other health and wellness components on Center. Key tasks include: 1. Providing medical services that emphasize early identification and diagnosis of medical problems. 2. Supervising Nurse Practitioners/Physician Assistants. 3. Conducting initial physical examinations and developing treatment plans. 4. Recording all medical documentation accurately. 5. Treating minor conditions and providing first aid. 6. Collaborating with other health professionals for comprehensive care. 7. Reporting health-related matters to the Center Director monthly. 8. Ensuring patient privacy in accordance with HIPAA regulations. Period of Performance The contract will be performed on a weekly basis during mutually agreed upon days and hours, typically one or two days a week from Monday to Friday between 8 AM and 5 PM.
