Background The Indian Health Service (IHS), specifically the Crownpoint Service Unit, is seeking to identify capable Indian-owned economic enterprises, both large and small, to provide laboratory supplies and reagents for the Crownpoint Healthcare Facility's Laboratory Department. Work Details The contractor will be responsible for providing a comprehensive range of laboratory supplies and reagents on an as-needed basis. They are required to submit a company profile that includes UEI, name and address, primary point of contact information, business size and type of ownership, socio-economic status/participation (such as 8(a), WOSB/EDWOSB, SDVOSB, HUBZone), identification of Native American ownership, representations and certifications. Additionally, ISBEEs and IEEs must complete an IHS IEE Representation Form in accordance with HHSAR 326.606-1(b). All bidders must also be registered in the System for Award Management at the time of submission and award.
