The Indian Health Service, Crownpoint Service Unit is issuing this sources-sought notice (SSN) to identify capable Indian-owned, economic enterprises large (IEE) or small (ISBEE) that can provide laboratory supplies, and reagents on an-as-needed basis for the Crownpoint Healthcare Facility, Laboratory Department. The Government anticipates establishing a Blanket Purchase Agreement (BPA), or other appropriate commercial supply vehicle for the acquisition of laboratory supplies, reagents, specimen collection materials, controls, testing kits, consumables, and related laboratory products required to support patient care and diagnostic services. The contractor should be able to provide a full-line laboratory supply catalog and be capable of furnishing a broad range of laboratory supplies and reagents as required by the Government. Supplies will be ordered on an as-needed basis throughout the ordering period. Other capable firms are also invited to respond to this SSN. The Contracting Officer will consider responses for Buy Indian Act (25 U.S.C. 47) opportunities first, then for set aside programs under the Small Business Act (41 U.S.C. 3104). The applicable North American Industry Classification System (NAICS) code assigned to this requirement is 325413 In-Vi65tro Diagnostic Substance Manufacturing with a small business size standard of 1250 employees. Information collected from this notice is purely for market research only and will be used to determine the contracting office's acquisition strategy including competition, set asides, and locating Indian-owned economic enterprises. Your submission must include the following information, at a minimum: - Company Profile that includes: UEI, Name and Address, Primary POC information, business size and type of ownership, socio-economic status/participation such as 8(a), WOSB/EDWOSB, SDVOSB and HUBZone, identification of Native American ownership, SAM.gov representations and certifications, and other related documentation. - A brief statement of the company's capability that relates to the described government requirement. Additional demonstration of capability may include current and past performance history for the same or similar requirement, established contracts such as GSA or VA Schedule, etc. - Indian-owned Small Business Economic Enterprises (ISBEEs) and Indian-owned Economic Enterprises (IEEs) must also submit a completed IHS IEE Representation Form (attached) in accordance with HHSAR 326.606-1(b). They are required to submit a company profile that includes UEI, name and address, primary point of contact information, business size and type of ownership, socio-economic status/participation (such as 8(a), WOSB/EDWOSB, SDVOSB, HUBZone), identification of Native American ownership, representations and certifications. Additionally, ISBEEs and IEEs must complete an IHS IEE Representation Form in accordance with HHSAR 326.606-1(b). All bidders must also be registered in the System for Award Management at the time of submission and award. Incumbent Analysis (see Incumbents section for more detail) This opportunity appears to have a confirmed incumbent and/or closely related prior contract based on BPA 75H71223A00008, awarded to Hardy Diagnostics for $568,606 (06/20/23–06/20/28). The prior award provided laboratory consumables, reagents, media, and test kits for the Phoenix Indian Medical Center laboratory under a BPA, with supplies used for clinical specimen testing and organism recovery/isolation. The scope described for the Crownpoint Healthcare Facility—full-line laboratory supplies and reagents on an as-needed basis to support patient care and diagnostic services—appears to be a continuation of the same type of lab supply/reagent replenishment function under a similar BPA framework.
