Background The Utah Department of Health and Human Services (DHHS) is seeking qualified Offerors to provide comprehensive audiology services to inmates at the Utah State Correctional Facility (USCF) and Central Utah Correctional Facility (CUCF). The goal of this contract is to ensure that inmates with hearing loss receive necessary support to participate in correctional programs and activities. The contract will be for a period of five years. Work Details The Contractor shall provide all-inclusive audiology services, including: 1. Basic hearing evaluations, including air, bone, speech, and discrimination tests. 2. Comprehensive audiometric examinations and tympanometry. 3. Acoustic reflex testing and pure tone audiometry. 4. On-site fitting, maintenance, and minor repairs of hearing aids for inmates diagnosed with moderate or significant hearing loss. 5. Submission of inmate test results and recommendations directly to DHHS after evaluations are completed. 6. Delivery of hearing aids within 7-10 days of order placement, with on-site repairs at no additional charge. 7. Access to the DHHS Electronic Health Record System for patient information review as needed. Period of Performance The contract will be performed over a period of five years from the date of award. Place of Performance Services will be provided at the Utah State Correctional Facility located at 1480 N 8000 W Salt Lake City, UT 84116 and Central Utah Correctional Facility located at 255 E 300 N St Gunnison, UT 84634. Bidder Requirements Offerors must provide certified Audiologists or Hearing Instrument Specialists with valid licenses from the Utah Division of Professional Licensing. They must also submit copies of these licenses annually to DHHS and notify DHHS immediately of any disciplinary actions affecting their licenses.
