HIV Case Management - St. Louis Region

Background

The State of Missouri, through the Department of Health and Senior Services, is issuing this Request for Proposal (RFP) for HIV Case Management services in the St. Louis Region. The purpose of this contract is to provide healthcare and supportive services to low-income individuals living with HIV/AIDS who lack access to appropriate care and treatment. The Missouri Department of Health and Senior Services has developed an HIV Case Management System aimed at assisting individuals in gaining and maintaining access to treatment, which includes medical, social, community, legal, and financial services.

Work Details

The contractor shall provide comprehensive HIV case management services including: 1. Collaboration with Missouri Ryan White grant recipients across designated counties (Franklin, Jefferson, Lincoln, St. Charles, St. Louis, Warren, and the City of St. Louis). 2. Coordination of care to ensure efficient service delivery without duplication. 3. Provision of interpretative language services for clients with limited English proficiency. 4. Compliance with minimum experience qualifications including five years of experience in community health partnerships and chronic disease projects. 5. Personnel qualifications for Direct Enrollment Specialists and HIV Case Managers must include relevant degrees or prior approval from the state agency. 6. Implementation of a health equity plan to ensure equitable access to care for all populations affected by HIV/AIDS. 7. Participation in monthly regional case management meetings and training programs as mandated by the state agency.

Period of Performance

The contract will be effective from the date of award through one year with options for renewal for two additional one-year periods.

Place of Performance

Services will be performed in the St. Louis Region of Missouri, specifically covering counties including Franklin, Jefferson, Lincoln, St. Charles, St. Louis, Warren, and the City of St. Louis.

Bidder Requirements

Vendors must be registered in MissouriBUYS with 'Prospective' or 'Spend Authorized' status to submit proposals. They must also comply with minimum experience qualifications outlined in Section 2: Scope of Work and maintain necessary licenses as required by the state agency.
