Background

The Commonwealth of Kentucky's Cabinet for Health and Family Services (CHFS), specifically the Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID), is issuing this Request for Proposals (RFP) to secure services from a qualified contractor to operate a HIPAA and 42 CFR Part 2 compliant virtual platform. This initiative aims to deliver evidence-based transitional care coordination services via telehealth for individuals with substance use disorders (SUD) and co-occurring mental health conditions. The target populations include those with opioid use disorder (OUD) and stimulant use disorder (StimUD), particularly pregnant or postpartum individuals, transition-age youth, and those involved in the criminal justice system. The overarching goal is to enhance care continuity and access to high-quality treatment across Kentucky, thereby reducing overdose deaths and improving health outcomes.

Work Details

The contractor will be responsible for: 1. Operating a HIPAA-compliant virtual platform for transitional care coordination through telehealth. 2. Conducting comprehensive screenings and assessments using evidence-based tools like the American Society of Addiction Medicine (ASAM) Criteria. 3. Delivering or coordinating services such as counseling, peer support, and system navigation for individuals with SUD. 4. Providing or coordinating access to FDA-approved medications for SUD treatment, including MOUD. 5. Maintaining eligibility as a Medicaid provider with all Managed Care Organizations (MCOs). 6. Establishing an interdisciplinary team that includes prescribers, counselors, peer support specialists, and care navigators. 7. Referring at least 50 individuals each month and conducting timely clinical screenings or assessments based on urgency levels.

Period of Performance

The contract will commence upon award and is expected to continue for a period defined by the terms of the RFP, with ongoing monthly reporting requirements.

Place of Performance

Services will be provided statewide across Kentucky via telehealth platforms.

Bidder Requirements

Bidders must demonstrate previous experience in providing or coordinating services related to prescribing, counseling, system navigation, or peer support services. They must also show capacity to serve priority populations identified in the RFP and comply with all applicable state and federal laws regarding service delivery.
