IVH Hospice Care

Background

The Indiana Veterans’ Home (IVH) is a state-owned long-term care facility located in West Lafayette, IN. It provides skilled nursing care, rehabilitation, and memory care services to honorably discharged Indiana veterans and their families. The IVH seeks to establish relationships with hospice providers to enhance the care provided to its residents, ensuring compliance with state and federal regulations regarding hospice services.

Work Details

The contract involves providing comprehensive hospice services that include: - Nursing care supervised by registered nurses; - Medical social services; - Physician services not covered by the attending physician; - Counseling services (bereavement, dietary, spiritual); - Physical, respiratory, occupational, and speech therapy; - Home health aide/homemaker services; - Medical supplies; - Drugs and biologicals; - Use of medical appliances; - Overall management of hospice patients. The hospice provider will have sole authority for patient admissions and discharges, assessments of eligibility for hospice services, and must coordinate care with the facility's staff. Deliverables include a Care Coordination Plan due 10 days after contract start, a Training Completion Report due 30 days after start, monthly progress reports, and a final transition package at the end of the contract.

Period of Performance

The contract will be executed for a period of five years from the date of execution with an option for one additional year at the State's discretion.

Place of Performance

The services will be performed at the Indiana Veterans’ Home located in West Lafayette, Indiana.

Bidder Requirements

Bidders must be registered with the Indiana Department of Administration and in good standing with the Secretary of State. They must comply with HIPAA regulations regarding Protected Health Information (PHI) and provide evidence of financial stability. Additionally, bidders are required to have a formal business continuity plan.
