The Illinois Secretary of State is seeking bids from qualified vendors for the printing of Secure Power of Attorney Forms, specifically form number VSD 332.5. 26 lb. white carbonless protective safety paper with invisible fluorescent fibers - Parts II, III, & IV: 15 lb. white carbonless paper Inks must include red PMS 199U and black for the face with security features such as a pantograph safety feature and a special patterned patch printed in fluorescent ink. All printing must be done in one pass through the press with strict quality control measures in place throughout production. The forms are to be packed in bundles of 50 and shipped in corrugated cartons that meet specified strength requirements. Delivery is expected by July 15, 2026, with proof required prior to final production.
The contract will commence upon execution and is set to run until June 30, 2027, with potential renewal options extending up to a total maximum term of ten years.
