Work Details

The vendor is required to print 2,000,000 copies of the Secure Power of Attorney Form (VSD 332.5). The forms must be four-part carbonless, top stub, snap-out style with specific dimensions: overall size of 8 1/2" wide and 11 5/8" high, and torn-out size of 8 1/2" wide and 11" high. The paper specifications include:

Part I

26 lb. white carbonless protective safety paper with invisible fluorescent fibers - Parts II, III, & IV: 15 lb. white carbonless paper Inks must include red PMS 199U and black for the face with security features such as a pantograph safety feature and a special patterned patch printed in fluorescent ink. All printing must be done in one pass through the press with strict quality control measures in place throughout production. The forms are to be packed in bundles of 50 and shipped in corrugated cartons that meet specified strength requirements. Delivery is expected by July 15, 2026, with proof required prior to final production.

Period of Performance

The contract will commence upon execution and is set to run until June 30, 2027, with potential renewal options extending up to a total maximum term of ten years.

Place of Performance

The forms will be printed at the vendor's facility; however, delivery will be made to the Office of the Secretary of State's Vehicle Services Department located at 3701 Winchester Road, Springfield, IL.
