The purpose of this contract is to provide a ready source of technical assistance (TA) to RHTP subgrantees and potential subgrantees seeking guidance and support related to RHTP-funded HIT tools and how implementation of these tools can help subgrantees achieve Minnesota's RHTP goals.
Background The Minnesota Department of Health (MDH) is initiating the Rural Health Transformation Program (RHTP) to provide technical assistance (TA) to rural health care providers in Minnesota. The contract aims to support RHTP subgrantees and potential subgrantees in implementing health information technology (HIT) tools that align with Minnesota’s RHTP goals. This initiative is funded by the Centers for Medicare & Medicaid Services (CMS) as part of a financial assistance award totaling $193,090,618.14. Work Details The contractor will provide various types of technical assistance to RHTP subgrantees and potential subgrantees, including: 1. Evaluating existing HIT systems and identifying gaps in their usage. 2. Advising on existing HIT support contracts and vendor capabilities. 3. Ensuring recommendations align with state and federal policies. 4. Assisting in planning for RHTP-funded opportunities and demonstrating measurable progress towards RHTP goals. 5. Building awareness of available technology funded through RHTP. 6. Identifying new tools compatible with existing systems and advising on integration feasibility. 7. Documenting key findings to support procurement of new technology or IT support. 8. Planning realistic deployment timelines for HIT investments. 9. Supporting full implementation of HIT tools to improve operational efficiencies and patient care. 10. Engaging regularly with MDH about services provided and submitting monthly narrative summary reports detailing TA provided, personnel involved, hours spent, progress made by subgrantees, and challenges faced. Period of Performance The anticipated contract term is from June 2026 through September 2027, with an option to extend for up to an additional 3.5 years. Place of Performance The services will be performed within the State of Minnesota, specifically targeting rural health care providers.
