Background The State of Delaware Department of Health and Social Services, Division of Developmental Disabilities Services, seeks professional services from vendors to provide home and community-based services (HCBS) for individuals with intellectual and developmental disabilities, including brain injury, autism spectrum disorder, or Prader-Willi Syndrome. This RFP is issued pursuant to 29 Del. C. §§ 6981 and 6982. Work Details Vendors must provide a range of home and community-based services as follows: - Assistive Technology - Behavior Consultation - Benefits Counseling - Career Assessment and Exploration - Community Participation - Community Transition - Day Habilitation - Financial Coaching - Home Accessibility/Adaptations - Medical Residential Habilitation - Nurse Consultation - Orientation and Mobility - Personal Care - Prevocational Services - Residential Habilitation (including Shared Living Agency) - Respite Care - Specialized Medical Equipment and Supplies - Supported Employment (Individual and Small Group) - Supported Living - Vehicle Accessibility/Adaptations. Vendors must agree to accept the established fee-for-service rates as a condition for being awarded a contract; these rates are published annually on the DDDS website. Period of Performance The contract term is for one year with four optional one-year extensions. The estimated notification of award is on September 1, 2026. Place of Performance Services will be performed within the State of Delaware. Bidder Requirements Vendors must possess necessary Delaware licenses and/or certifications to perform the required services. Proof of insurance must be provided prior to the start of the contract period. Vendors must also submit a transmittal letter summarizing their interest in providing services and any exceptions taken to the RFP requirements. Additionally, vendors must complete all required attachments including proof of qualified provider letters from DDDS.
