Cancer Program Patient Navigator

Background

The State of Delaware, through its Department of Health and Social Services, is seeking to engage a vendor to provide professional services related to the Cancer Program Patient Navigator. The goal of this contract is to enhance the support and navigation services for patients undergoing cancer treatment, ensuring they receive comprehensive care and assistance throughout their treatment journey.

Work Details

The vendor will be responsible for providing the following services: 1. Patient navigation services that include assisting patients in understanding their diagnosis, treatment options, and available resources. 2. Coordination with healthcare providers to ensure continuity of care. 3. Development of educational materials tailored for patients and their families regarding cancer treatment and support resources. 4. Regular reporting on patient engagement and outcomes as specified in the Statement of Work (SOW) attached as APPENDIX XX.

Period of Performance

The contract will commence on the START DATE and will continue until the END DATE, with options for renewal based on performance and funding availability.

Place of Performance

Services will be performed primarily within the State of Delaware, with potential outreach to patients in various healthcare settings across the state.

Bidder Requirements

Vendors must possess a valid Delaware business license as per 30 Del. C. § 2101. Additionally, vendors are required to maintain compliance with all applicable federal, state, and local laws, including HIPAA regulations concerning patient confidentiality and data protection.
