Background The State of Delaware, through its Department of Health and Social Services, is seeking to engage a vendor to operate high-need elementary school-based health centers. The goal of this contract is to provide essential health services to students in Delaware's elementary schools, ensuring access to healthcare and promoting the overall well-being of children in these communities. Work Details The vendor will be responsible for providing comprehensive health services at designated elementary school-based health centers. This includes but is not limited to: - Medical assessments - Mental health services - Preventive care - Health education - Coordination with parents and guardians. The vendor must ensure compliance with all applicable federal and state laws regarding health services and maintain the confidentiality of student health records as per HIPAA regulations. Additionally, the vendor will be required to submit monthly reports detailing service delivery metrics and outcomes. Period of Performance The contract will commence on the effective date specified in the agreement and will continue for a period of three years, with options for renewal based on performance and funding availability. Place of Performance Services will be performed at various elementary schools throughout the State of Delaware as designated by the Department of Health and Social Services.
