THIS IS NOT A SOLICITATION. Information collected from this notice is purely for market research only and will be used to determine the contracting office's acquisition strategy including competition, set asides, and locating Indian-owned economic enterprises. Submission of any information in response to this market survey is purely voluntary. The government assumes no financial responsibility for any costs incurred in response to this request for information. In accordance with FAR 4.1102(a), offerors and quoters are required to be registered in System for Award Management at SAM.gov, at the time an offer or quotation is submitted in order to comply with the annual representations and certifications requirements.

Interested parties who consider themselves qualified to perform the above-listed services are invited to submit a response to this notice by March 16, 2026 @ 10:00 am (MST) to Daisha Richards at Daisha.Richards@ihs.gov with the subject line SSN KHC Modification of security Booth, IHS1523318 Your submission must include the following information, at a minimum: - Company Profile that includes: UEI, Name and Address, Primary POC information, business size and type of ownership, socio-economic status/participation such as 8(a), WOSB/EDWOSB, SDVOSB and HUBZone, identification of Native American ownership (if applicable), SAM.gov representations and certifications, and other related documentation. - A brief statement of the company's capability that relates to the described government requirement. Additional demonstration of capability may include current and past performance history for the same or similar requirement, established contracts such as GSA or VA Schedule, etc. - If claiming Indian-owned Small Business Economic Enterprise (ISBEE) or Indian-owned Economic Enterprise (IEE), submit completed IHS IEE Representation Form (attached). Background The Indian Health Service, Kayenta Service Unit is conducting market research to identify prospective sources for the modification of an existing 6' by 10' Guardian Booth. The Kayenta Health Center serves a population of approximately 20,000 people in a remote area of Arizona, providing essential health services including walk-in clinics and a 24/7 emergency room. The goal of this contract is to enhance security measures at the facility. Work Details The contractor will be responsible for the following tasks: 1. Replace the existing guard shack with a bullet-proof security guard booth. 2. Install crash-proof steel bollards around the booth. 3. Specific modifications include: - Furnishing and installing a 6 feet by 10 feet bullet-proof security guard booth with Level 3 ballistic proof glass windows, doors, and panels. - Removing the existing Guardian booth and ensuring it has a main electrical disconnect. - Removing and reconnecting electrical power to the new booth. - Providing all necessary supplies and materials for project completion. 4. For bollard installation: - Furnish and install eight steel bollards surrounding the booth, dug to a depth of 3 feet or as required by ASTM standards, with concrete poured around them. - Bollards must be capable of stopping a 5000 lb vehicle at 30 mph (M30) and must be removable.
