The following draft performance work statement is for Testing, Pick up and Disposal of Petroleum, Oil and Lubricant (POL) from various Army Installations on the Island of Oahu, State of Hawaii. The purpose of this sources sought notice is to determine interest and capability of potential qualified firms relative to the North American Industry Classification (NAICS) code 562219. Please provide the following information on your capability statement: 1) Your intent to submit a quote for this requirement when it is formally advertised. 2) Name of firm with address, phone number, e-mail address, and point of contact. 3) CAGE Code and Unique Entity Identifier (UEI) 4) Size of firm including category of small business, such as 8(a), Women-Owned Small Business (WOSB), HUBZone, Small Disadvantaged Veteran Owned Small Business (SDVOSB); and 5) Statement of Capability stating your skills, experience, and knowledge required to perform the specific type of work. Background This Sources Sought Notice is issued by the Army to gather information on potential qualified firms for the Testing, Pick up and Disposal of Petroleum, Oil and Lubricant (POL) from various Army Installations on the Island of Oahu, State of Hawaii. Work Details The contractor will be responsible for implementing standard commercial practices for accident prevention and safety compliance while performing the requirements outlined in the performance work statement. Key tasks include: - Initiating an accident prevention and safety program upon commencement of work. - Conducting a safety orientation for all employees immediately after their employment. - Providing all necessary occupational health services to employees. - Ensuring contractor employees report any occupational health hazards to the Project Manager. - Complying with OSHA regulations and Hawaii Occupational Safety and Health Law. - Reporting all occupational injuries and illnesses as per Federal OSHA requirements, including completing necessary forms such as DD Form 689 and DA Form 285. - Maintaining required insurance coverage during contract performance, including comprehensive general liability, automobile liability, and workers' compensation insurance.
