Background The State of Delaware, through its Division of Public Health, seeks to engage a Vendor to provide professional services under a contract titled HSS-XX-XXX. This contract is aimed at fulfilling specific health-related services that align with the mission of the Division of Public Health, which focuses on improving the health and well-being of Delaware residents. Work Details The Vendor shall perform services as specified in the attached Appendices, which include but are not limited to: 1) Professional quality and technical accuracy in service delivery; 2) Compliance with standards set by the Department of Technology and Information (DTI); 3) Submission of monthly invoices detailing services provided; 4) Adherence to a project schedule outlined in Appendix XX; 5) Cooperation with Delaware's representatives for consultation and data sharing; 6) Maintenance of all public records related to this Agreement as per Delaware law. The Vendor must also ensure that all employees assigned to this project comply with any health mandates issued by the State during emergencies. Period of Performance The initial term of the contract will commence on the Start Date and will continue until the End Date, with options for renewal for additional periods as specified in amendments between the Vendor and Delaware. Place of Performance Services will be performed primarily within the State of Delaware, specifically at locations designated by the Division of Public Health. Bidder Requirements Vendors must possess a valid business license to operate in Delaware and comply with all applicable federal, state, and local laws. Additionally, Vendors must maintain insurance coverage including but not limited to: Worker’s Compensation, Commercial General Liability ($1 million per occurrence), Automotive Liability Insurance, and any necessary Medical/Professional Liability or Errors and Omissions coverage. A Unique Entity Identifier (UEI) registration is required for federal grants.
