Background The State of Delaware Department of Health and Social Services, Division of Substance Abuse and Mental Health, is seeking proposals for professional services to provide Personal Care services under the PROMISE program. This initiative aims to enhance the community-based behavioral health system for individuals with severe and persistent mental illness (SPMI) and significant substance use disorders (SUDs). The PROMISE program offers home- and community-based services that are person-centered and recovery-oriented, supporting beneficiaries in living independently within their communities. Work Details The selected vendor will provide Personal Care services to adults aged 18 and older who are residents of Delaware, have SPMI, and require support to live safely in the community. Services must be tailored to individual needs as outlined in a Recovery Plan developed collaboratively with beneficiaries. The scope includes: - Assisting with daily living activities such as personal hygiene, meal preparation, and medication management. - Providing transportation for shopping, appointments, and other personal care needs. - Ensuring that all services are delivered in a manner that promotes recovery and independence. - Utilizing an Electronic Visit Verification (EVV) system as per federal requirements. Period of Performance The contract term is two years with three optional one-year extensions. Place of Performance Services will be provided throughout the State of Delaware. Bidder Requirements Vendors must possess necessary Delaware licenses or certifications to perform the required services. Proof of insurance is required prior to contract commencement, including coverage for general liability, automotive liability, and professional liability. Vendors must also comply with background check requirements for employees interacting with vulnerable populations.
