Background

The Department of Veteran Affairs, Network Contracting Office 7 (NCO7), is issuing a solicitation for Community Nursing Home (CNH) Services for the Joseph Maxwell Cleland Atlanta VA Medical Center. This acquisition aims to provide essential nursing home services to veterans in the Atlanta VAMC catchment area, which includes multiple counties. The solicitation will be open to all businesses.

Work Details

The contractor will furnish all supplies and services at per diem rates for specified Levels of Care. Nursing home facilities must ensure care meets health needs and promotes the well-being of VA patients, covering total medical, nursing, and psychosocial needs. Facilities must have current CMS certification and state nursing home license. Key requirements include: - Ability to accept VA referrals ideally within 24 hours. - Monthly provider visits. - Availability of laboratory, x-ray, and other special services as needed. - Inclusion of room, meals, nursing care, and other necessary services without extra charge. - Quality oversight visits by VA representatives to ensure continuity of care. - Billing under the Patient Driven Payment Model (PDPM) following the Prospective Payment System (PPS) guidelines.

Period of Performance

The period of performance for each Indefinite Delivery Indefinite Quantity (IDIQ) contract will be based on the award date and will continue for 5 years.

Place of Performance

Services will be performed in the Atlanta VAMC catchment area which includes the following counties: Towns, Union, Gordon, Pickens, Dawson, Lumpkin, Floyd, Bartow, Cherokee, Forsyth, Hall, Banks, Polk, Paulding, Cobb, Gwinnett, Barrow, Jackson, Haralson, Carroll, Douglas, Fulton, Dekalb, Rockdale, Walton, Oconee, Clarke, Heard, Coweta, Troup, Meriwether, Pike, Upson, Lamar, Spaulding, Butts, Jasper, Newton, Morgan, Clayton, Fayette and Henry counties.
