Background The John F. Kennedy Day School, located on the Fort Apache Indian Reservation in Cedar Creek, Arizona, provides educational services to students with disabilities. Work Details The contractor shall provide Occupational Therapy (OT) services including: direct in-person therapy to students, consultations with staff, screenings, evaluation reports, instructions/directions to teaching assistants, participation in Individualized Education Program (IEP) meetings, and parent communication regarding questions or concerns. The contractor will also enter quarterly progress reports into the Native American Student Information System (NASIS), including evaluation summaries and present levels into IEPs. A direct service log for each student visit must be maintained. The contractor is expected to meet with the Special Education Department at the beginning of the contract and periodically thereafter to discuss student progress. Approximately 6 students will require bi-weekly OT services, necessitating the contractor's presence at the school for 2 days each month.
