Q402- Atlanta VAMC Community Nursing Home Services

Background

The Department of Veterans Affairs (VA) is issuing this solicitation for Community Nursing Home (CNH) services within the Atlanta VAMC catchment area. The goal of this contract is to provide comprehensive nursing care that meets the health needs and promotes the well-being of VA patients. The CNH program is essential for ensuring that eligible veterans receive appropriate care in skilled nursing facilities, which must be certified by the Center for Medicare and Medicaid Services (CMS) and possess a valid state nursing home license.

Work Details

The contractor will be responsible for furnishing all supplies and services necessary to provide nursing home care at per diem rates for specified Levels of Care. This includes: - Ensuring that care meets the medical, nursing, and psychosocial needs of VA beneficiaries. - Providing timely acceptance of VA referrals, ideally within 24 hours. - Offering monthly provider visits and necessary laboratory, x-ray, and special services. - Delivering room, meals, nursing care, and other services without additional charges as per VA-authorized levels of care. - Conducting quality oversight visits by authorized VA representatives to ensure continuity of care and assist veterans' transitions back into the community when applicable. The contract anticipates awarding multiple Indefinite Delivery Indefinite Quantity (IDIQ) contracts with values ranging from $750,000 to $3,000,000 based on estimated needs in specific counties.

Period of Performance

The period of performance for each IDIQ contract will begin on the award date and continue for five years.

Place of Performance

Services will be performed in various counties within the Atlanta VAMC catchment area including Towns, Union, Gordon, Pickens, Dawson, Lumpkin, Floyd, Bartow, Cherokee, Forsyth, Hall, Banks, Polk, Paulding, Cobb, Gwinnett, Barrow, Jackson, Haralson, Carroll, Douglas, Fulton, Dekalb, Rockdale, Walton Oconee Clarke Heard Coweta Troup Meriwether Pike Upson Lamar Spaulding Butts Jasper Newton Morgan Clayton Fayette and Henry counties.
