Background The Department of Veterans Affairs (VA) is issuing this contract to provide Health Care for Homeless Veterans (HCHV) Emergency Residential Services in Eureka, Humboldt County. The mission of the VA is to assist homeless Veterans in accessing essential services, including shelter, medical care, and case management, to improve their overall health and facilitate their transition to permanent housing. Work Details The Contractor shall provide short-term emergency housing for eligible homeless Veterans enrolled at the San Francisco VA Health Care System. Specific tasks include: 1. Providing 15 beds per day on a per diem basis for homeless Veterans. 2. Delivering meals and case management services to support Veterans in accessing medical care and other resources. 3. Developing individualized care plans within 14 days of admission that include goals for achieving stable housing and assessments of strengths and barriers. 4. Conducting weekly individual case management meetings focusing on each Veteran's needs, including assistance with permanent housing searches and increasing income through various financial resources. 5. Providing therapeutic and rehabilitative services, including mental health treatment and substance use disorder support using evidence-based practices. Period of Performance The contract period is from September 1, 2026, to August 31, 2031, with five ordering periods spanning one year each. Place of Performance Eureka, California. Bidder Requirements Bidders must demonstrate experience providing services to homeless veterans and must have qualified personnel available for the program's operational needs.
