Background

The Agency of Human Services (AHS), Department of Health (VDH) is issuing a funding opportunity through Vermont’s Rural Health Transformation Program (RHTP). The RHTP aims to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce, and ensure that Vermonters receive timely and affordable care. Vermont has received a grant of $195 million for Year One of a five-year initiative to address significant financial and operational strains in the healthcare system, focusing on improving care transitions and supporting patients with complex needs.

Work Details

The contract involves implementing Mobile Integrated Healthcare (MIH) services at five geographically distributed sites across rural Vermont. Each site must provide at least one of the following core MIH services: 1. Congestive Heart Failure (CHF) 2. Chronic Obstructive Pulmonary Disease (COPD) 3. Joint Replacement Care. The selected MIH sites will be responsible for delivering active service implementation, which includes: - Defined schedule of patient contact following hospital discharge - In-home clinical assessment and treatment - Medication reconciliation and adherence review - Review of discharge instructions - Disease-specific education - Communication with referring providers regarding in-home findings - Identification of barriers impacting recovery. Funding may be used for personnel costs, education and training, vehicles, IT, operational equipment, supplies, and medications directly associated with service delivery.

Period of Performance

The grant period is anticipated to start on August 1, 2026, and extend through September 30, 2027, for a total duration of 14 months. Year 1 funds must be fully spent by September 30, 2027.

Place of Performance

The contract will be performed in rural areas across Vermont, specifically excluding Chittenden County unless specific criteria are met.

Bidder Requirements

Applicants must be either a Vermont state licensed EMS agency with a formal clinical partnership with a sponsoring hospital or healthcare facility or a hospital deploying MIH services through internally employed EMS personnel. Preference will be given to rural locations. Applicants must demonstrate operational readiness or near-operational readiness as defined in the NOFO.
