Background

The Agency of Human Services, Department of Vermont Health Access is issuing this Request for Proposal (RFP) as part of a $195 million Rural Health Transformation (RHT) grant received by Vermont. The goal of the grant is to enhance rural health networks, improve technology and shared operations, and strengthen the rural health workforce to ensure that Vermonters receive timely and affordable healthcare.

Work Details

The selected vendor(s) will provide workforce support and system development through training and continuing education programs. The scope includes: 1) Development and delivery of training sessions on various health topics such as Diabetes, COPD, Mental Health/SUD, Hypertension, Asthma, Congestive Heart Failure (CHF), and Patient/Family Engagement. 2) Ensuring Continuing Education Units (CEUs) for participating health professionals. 3) Administration of post-educational assessments to evaluate training effectiveness. 4) Coordination with the Blueprint for Health team for planning meetings and training sessions. All training must align with evidence-based best practices and be accessible to comply with federal regulations.

Period of Performance

The contract period is anticipated to begin on August 1, 2026, and extend through September 30, 2027. There may be options for up to four one-year extensions based on performance and funding availability.

The services will be performed within the State of Vermont, with specific locations for in-person training sessions requiring prior approval from the Blueprint for Health team.
