Background

The Agency of Human Services, Department of Vermont Health Access is issuing this Request for Proposal (RFP) as part of a $195 million Rural Health Transformation (RHT) grant received in Year One. The goal of the grant is to enhance rural health networks, improve technology and shared operations, and strengthen the rural health workforce to ensure that Vermonters receive timely and affordable care. This RFP seeks contractors to provide Clinical Knowledge Translation to support the Blueprint for Health’s workforce development goals.

Work Details

The contractor will conduct a comprehensive review of existing clinical practice guidelines and recommend state-endorsed guidelines for conditions such as diabetes, hypertension, COPD, CHF, and mental health/substance use disorders. The work includes: 1. Evidence-Based Guideline Review: Ensure guidelines reflect best practices suitable for Vermont's healthcare settings. 2. Development of State-Endorsed Clinical Care Pathways: Collaborate with stakeholders to create standardized care pathways that include screening protocols, risk stratification approaches, and integration strategies for mental health and substance use disorders. 3. Development of Clinical Workflows and Implementation Tools: Create diagrams, workflows, referral processes, and patient management algorithms tailored for primary care teams. 4. Measurement Framework Development: Establish metrics for evaluating implementation and outcomes including guideline adoption rates and clinical outcome measures.

Period of Performance

The contract period is anticipated to start on August 1, 2026, and extend through September 30, 2027. Contracts may be extended for up to four additional one-year periods with State approval.

Place of Performance

The services will be performed in Vermont's primary care, specialty care, and Community Health Team settings.

Bidder Requirements

Bidders must comply with minimum qualifications outlined in the RFP. They are required to have experience relevant to the scope of work described in the RFP. Additionally, bidders must be registered with the Vermont Secretary of State’s office and obtain a Contractor’s Business Account Number from the Vermont Department of Taxes.
