Background

The Agency of Human Services, Department of Vermont Health Access is issuing this Request for Proposal (RFP) as part of a $195 million Rural Health Transformation (RHT) grant aimed at enhancing rural health networks in Vermont. The goal is to improve technology, strengthen the rural health workforce, and ensure that Vermonters receive timely and affordable care. This RFP focuses on strengthening Blueprint's capacity to advance high-performing primary care practices that deliver coordinated, patient-centered, and population-focused care.

Work Details

The contractor will implement the Clinical Champions component of the strategy, which includes hiring, training, and managing a network of Clinical Champions. These champions will provide clinical leadership and technical assistance to practices participating in the Blueprint program. Specific responsibilities include: - Providing on-site clinical technical assistance to participating practices. - Collaborating with Blueprint Quality Improvement Facilitators to support practice transformation efforts. - Assisting practices with workflow redesign and implementation of team-based care models. - Supporting evidence-based protocols for chronic disease management. - Guiding mental health and substance use disorder screening and treatment protocols. - Facilitating coordination between primary care and behavioral health providers. The contractor must ensure that Clinical Champions have diverse backgrounds including physicians, nursing professionals, mental health experts, and substance use disorder treatment specialists.

Period of Performance

The contract period is anticipated to begin on August 1, 2026, and extend through September 30, 2027. Contracts may be extended for up to four additional one-year periods with State and CMS approval.

Place of Performance

The services will be performed across all 13 Health Service Areas in Vermont.

Bidder Requirements

Bidders must demonstrate minimum qualifications including relevant clinical licensure and experience in team-based care, chronic disease management, integrated behavioral health/SUD care, practice transformation, population health or quality improvement leadership. Bidders must also comply with worker classification compliance requirements if bid amounts exceed $250,000.
