Background

The Agency of Human Services, Department of Vermont Health Access is issuing this Request for Proposal (RFP) as part of a Rural Health Transformation (RHT) grant totaling $195 million over five years. The goal is to enhance rural health networks, improve technology and operations, and strengthen the workforce to ensure that Vermonters receive timely and affordable healthcare. This RFP specifically seeks vendors to provide workforce support and system development through case manager training and certification programs for health professionals in Vermont.

Work Details

The contractor will provide case manager training aimed at certifying approximately 200 health professionals involved in healthcare within Vermont. The training will focus on chronic and complex conditions such as Hypertension, Diabetes, COPD, CHF, Mental Health, and Substance Use Disorders. Key components of the training include: - Care management strategies - Reimbursement methods - Addressing health-related social needs - Quality evaluation and outcome measurements - Ethical, regulatory, and practice standards - Hosting certification examinations. Additionally, the contractor must ensure Continuing Education Units (CEUs) are awarded to participants and conduct post-training evaluations to assess effectiveness. The contractor will also be responsible for quarterly reporting on metrics such as the number of unique individuals served.

Period of Performance

The contract period is anticipated to begin on August 1, 2026, and extend through September 30, 2027. There is potential for up to four one-year extensions based on performance and funding availability.

Place of Performance

The training sessions must be conducted within the State of Vermont. Specific locations for in-person sessions must be pre-approved by the Blueprint for Health at least 60 days prior to the event.

Bidder Requirements

Bidders must comply with specific qualifications including: - Registration with the Vermont Secretary of State. - Completion of a Certificate of Compliance regarding worker classification if bid amounts exceed $250,000. - Adherence to Executive Order 05-16 concerning climate change considerations in state procurements. - Disclosure of any conflicts of interest. Bidders are also required to submit a detailed proposal that includes a cover letter, technical response outlining their approach, references from similar projects completed in the last year (excluding state agencies), reporting requirements documentation, a price schedule, a signed Certificate of Compliance, and an AI Usage Disclosure Form.
