Concrete Removal/Installation in BLDG. 682 at Wichita Falls State Hospital

Background

The Texas Health and Human Services Commission (HHSC) is an agency within the Texas Health and Human Services (HHS) system. The Procurement and Contracting Services (PCS) division of HHSC administers Invitations for Bids (IFBs) for HHS. HHSC PCS is seeking competitive bids to establish contract(s) for concrete removal and installation in Building 682 at Wichita Falls State Hospital. Both HHS Agencies, HHSC and DSHS, will be entitled to use any contract awarded as a result of this IFB.

Work Details

The required services include concrete removal and installation in Building 682 at Wichita Falls State Hospital. The driveway leading to the kitchen loading dock has collapsed and is unusable for food service delivery. The contractor must: 1. Saw, cut, and remove all affected driveway areas. 2. Install four (4) sections of concrete parking. 3. Install a minimum of six (6) inch thick concrete. 4. Reinforcement shall be half an inch long, placed at eighteen (18) inch centers each way. Prior to performing any service, the contractor must coordinate with WFSH regarding staging areas for equipment, notify WFSH of all personnel performing services, and inspect work areas for pre-existing defects.

Period of Performance

The anticipated contract start date is upon award, with services to be performed Monday through Friday between the hours of 8 a.m. and 5 p.m. Central Time.

Place of Performance

Wichita Falls State Hospital, 6515 Kemp Blvd, Wichita Falls, TX 76308.

Bidder Requirements

Bidders must have relevant experience required for the performance of the services outlined in this IFB with a minimum experience requirement of three (3) years. Personnel assigned to perform the services must have at least two (2) years relevant experience. All personnel must be at least 18 years old and provide a minimum of three verifiable references for similar contracts within the last three years.
