Safety Glasses

Background

The Indiana Department of Health (IDOH) is seeking bids for a vendor to fabricate and supply prescription safety glasses for its laboratory employees. The goal of this contract is to establish a multi-year pricing agreement for these necessary safety items, ensuring that employees have access to appropriate eyewear based on their individual prescriptions.

Work Details

The vendor will be responsible for fabricating and supplying prescription safety glasses according to the specifications provided by IDOH. The glasses must be made per the employee's valid prescription, which will include standard safety frames and various lens types as outlined below: **Table 1 - Frames**: - Standard safety frames - Single SV – polycarbonate - Bifocal BF28 – polycarbonate - Trifocal 7X28 – polycarbonate - Progressive Comfort – polycarbonate - Side shields (safety side shields) - Power over standard if required on the prescription. **Table 2 - Power Over Standard (no PAL)**: Sphere Power and Cylinder specifications are detailed with specific ranges for different lens types, indicating the power requirements based on the employee's prescription. The vendor is expected to provide an estimated quote/pricing details for a duration of 5 years.

Period of Performance

The contract will commence upon award and is expected to span multiple years, with specific pricing agreements established for each year.

Place of Performance

The services will be performed in Indianapolis, Indiana, where the IDOH laboratory is located.

Bidder Requirements

Bidders must comply with all applicable regulations and certifications. They are encouraged to submit a Minority/Women's Business Enterprises (MWBE) Subcontractor Commitment Form if applicable, as there are participation goals of 4% for minority participation and 9% for women participation. Additionally, bidders must ensure they are registered with the Secretary of State and comply with all ethical requirements as set forth by Indiana law.
