The State of West Virginia has been awarded approximately $200 million through the Rural Health Transformation Program (RHTP), established under Section 71401 of Public Law 119-21, the One Big Beautiful Bill Act, signed into law by President Trump on July 4, 2025. These funds are intended to strengthen provider productivity, expand access to care, support workforce sustainability, and enable statewide value-based care adoption through integrated technology, operations, and data infrastructure across rural communities. This initiative aligns with Health and Human Services Secretary Robert F. Kennedy’s 'Making America Healthy Again' and Governor Morrisey’s 'Four Pillars of a Healthy West Virginia' goals by focusing on reducing chronic disease burdens and improving food quality. The RHTP consists of seven initiatives including Smart Care Catalyst and Provider Productivity Support Fund (PPSF), which aims to modernize rural healthcare by funding operational upgrades.
The Provider Productivity Support Fund (PPSF) will provide financial support to rural hospitals, clinics, and stand-alone provider offices to offset high clinical and administrative operating expenses. The funding will be directed towards: 1. Improving operational efficiency through modernization of technology and workflows such as revenue cycle modernization, billing automation, AI-based documentation tools, workflow automation, cybersecurity upgrades, and cost-optimization initiatives. 2. Supporting shared-service collaboratives that reduce costs and duplication. 3. Enabling successful adoption of Value-Based Care (VBC) arrangements. Funding is contingent upon measurable operational improvements tied to verified productivity gains. Grantees must demonstrate sustained use of funded tools or services and report on productivity metrics monthly. Proposals must include a clear measurement plan demonstrating improvements in provider productivity and VBC readiness within a defined timeframe.
Funding is for one year with all funds to be spent no later than July 31, 2027.
The contract will be performed in all counties in West Virginia with priority given to rural high-need regions that have a high prevalence of chronic conditions.
Eligible applicants include rural hospitals (including Critical Access Hospitals), Federally Qualified Health Centers (FQHCs), community health centers, rural clinics, independent primary care practices, behavioral health providers, EMS agencies, community paramedicine programs, and provider-led collaboratives. Applicants are encouraged to provide matching funds; preference will be given to those who can offer at least a 1:1 match. All applicants must demonstrate current participation or commitment to participate in value-based care arrangements.
