The goal of the HCHV program is to remove homeless veterans from the street or other places considered unfit for human habitation and place them in community-based, residential environments with sufficient on-site psychosocial support services to meet the needs of those veterans and place them in permanent housing as rapidly as possible. This program will be located in a designated building and floor, on-site at the Northport VAMC at 79 Middleville Road, Northport, NY 11768.
