Kenosha County is seeking qualified service providers to deliver comprehensive dementia and caregiver support services in alignment with the priorities and program frameworks established under the Division of Aging and Disability Services. The Kenosha County Department of Human Services (KCDHS) aims to develop, coordinate, and administer a comprehensive network of services to support individuals coping with dementia and their caregivers.
The contract includes the following core service components: 1. Dementia Care: - Support for individuals living with Alzheimer’s disease or related dementias, including training staff at local agencies to assist systems in becoming dementia-capable, helping communities become dementia-friendly, and providing education and support to families. - At least 1.0 FTE Dementia Care Specialists (DCS) must be hired, who will conduct memory screenings, provide referrals, and assist with care planning. 2. Caregiver Support: - Coordination of services for family members providing in-home care for older adults or individuals with disabilities. - At least 1.0 FTE caregiver support coordinator must be hired to manage caregiver needs assessments, provide information about resources, assist in accessing services, and offer counseling and respite services.
The contract is for a one-year term from January 1, 2027, to December 31, 2027, with options for four additional one-year renewals based on mutual agreement.
Services will be performed within Kenosha County, Wisconsin.
Proposers must have a Bachelor’s degree in a relevant field and at least one year of experience working directly with individuals affected by dementia or caregiving. They must also hold a valid Wisconsin driver’s license and have reliable transportation for in-home visits. Additionally, contractors must comply with various insurance requirements including worker's compensation, general liability insurance, professional liability insurance, and others as specified.
