Contacting any other individuals at Upstate regarding this advertisement may violate New York State Finance Law and could result in consequences ranging from no contract award to debarment from sales to any NYS Agency. - Vendors must complete and submit all applicable documents related to Vendor Responsibility, NYS Finance Law Sections 139 j & k, and New York State's Tax Law Section 5-a. - Vendors must also submit valid and current NYS Workers' Compensation and Disability Forms.
