Rural MotherInfant Welcome Home Initiative Support Services

Background

The Tennessee Department of Health (TDH) is initiating the Rural Mother-Infant Welcome Home Initiative to enhance maternal and infant health outcomes in rural areas. This initiative is part of TDH's Community Health Access and Navigation in Tennessee (CHANT) Program, which aims to connect pregnant and postpartum women, infants, children, and families with essential services such as care coordination, health education, and resource navigation. The goal is to improve access to education, supplies, community supports, and trusted local resources for families in designated rural counties.

Work Details

The services sought under this initiative include: - Event planning and logistics for community-based Mother-Infant Welcome Home events or baby showers; - Participant outreach and engagement strategies; - Coordination with TDH, CHANT staff, county health departments, and local partners; - Distribution of TDH-approved mother-infant registry items; - Delivery of maternal and infant health education; - Inventory tracking; - Documentation; - Reporting; - Related implementation support. Organizations are expected to provide a comprehensive service model that may involve direct service provision or collaboration with subcontractors.

Period of Performance

The contract will be executed following the completion of the RFI process, with implementation timelines to be determined based on organizational readiness and project requirements.

Place of Performance

Services will be performed in designated rural counties across Tennessee as identified by the Tennessee Department of Health.

Bidder Requirements

Organizations responding to this RFI should demonstrate experience in supporting maternal health, infant health, family engagement, rural health initiatives, public health outreach, event coordination, and similar services. They should also outline their capacity to operate in rural communities and provide a culturally responsive approach to service delivery.
