Statewide Dental Services

Background

The Florida Agency for Persons with Disabilities (APD) is responsible for providing services to individuals with disabilities. The goal of this contract is to establish a framework for the provision of statewide dental services to eligible clients, ensuring they receive necessary dental care in accordance with state regulations and standards.

Work Details

The Provider shall deliver quantifiable, measurable, and verifiable units of dental services as specified in the contract. Each service must relate directly to the scope of work and include performance measures. Deliverables must be accepted by the Contract Manager prior to payment. The Provider is required to submit detailed bills for services rendered, allowing for proper auditing. Additionally, the Provider must maintain public access to all records related to the contract, except where confidentiality is mandated by law. Compliance with state financial assistance regulations is required, ensuring that all expenditures are documented and monitored.

Period of Performance

The contract will commence upon signing by both parties and will continue until terminated or completed as per the terms outlined in the contract.

Place of Performance

Services will be provided statewide across Florida, specifically targeting areas where eligible clients reside.

Bidder Requirements

The Provider must comply with various statutory requirements including maintaining adequate liability insurance coverage throughout the duration of the contract. Providers must not be on any Convicted Vendor List or Discriminatory Vendor List as per Florida statutes. Additionally, Providers are required to adhere to civil rights laws and ensure no discrimination occurs in service delivery.
