Statewide Dental Services

Background

The Florida Agency for Persons with Disabilities (APD) is issuing this contract to provide statewide dental services aimed at supporting individuals with disabilities. The goal is to ensure that clients receive necessary dental care in a manner that is accessible and compliant with state regulations.

Work Details

The Provider shall deliver quantifiable, measurable, and verifiable units of dental services as specified in the contract. Each service must be directly related to the scope of work and include performance measures. Services include but are not limited to: - Routine dental examinations - Preventive care - Restorative procedures - Emergency dental services - Necessary follow-up treatments The Provider must submit detailed bills for fees or other compensation for services rendered, ensuring compliance with Florida Statutes regarding public records and financial documentation.

Period of Performance

The contract will commence upon signing by both parties and will continue until all obligations are fulfilled or terminated as per the terms outlined in the contract.

Place of Performance

Services will be provided statewide across Florida, specifically targeting areas where individuals with disabilities reside.

Bidder Requirements

The Provider must comply with all applicable state laws, including those pertaining to public entity crimes and discriminatory contractors. The Provider must not be listed on the Convicted Vendor List or Discriminatory Vendor List. Additionally, adequate liability insurance coverage must be maintained throughout the duration of the contract.
