SUNY Upstate Medical University is seeking to purchase the following products or their equivalents: - PuraStat 3ml Syringe (621-062) - PuraStat Syringe Adapter (001-099) - PuraStat Catheter (WP-18/2200) Vendors capable of providing these items, or equivalent products, are invited to submit a proposal.

Background

The purchase is being made under SUNY Upstate's discretionary authority, as authorized by NYS Education Law 355(5)(a), which allows for purchases without a formal competitive process in certain circumstances. No sealed bid will be held for this solicitation.

Contract Requirements

- This procurement is subject to NYS Finance Law 139 j&k (the Lobbying Law). Vendors are cautioned that the only permissible contact for this procurement is the individual listed below. - The successful contractor must submit valid and current NYS Workman's Compensation and Disability Forms. - The successful contractor must complete and submit applicable documents related to Vendor Responsibility, NYS Finance Law 139 j&k, and New York State's Tax Law Section 5-a.
