Background

The City of Los Angeles, through its Department of General Services - Supply Services Division, is seeking quotations for Emergency Medical Kits and Supplies. This initiative aims to ensure that the city has adequate medical supplies readily available for emergencies, aligning with the city's commitment to public health and safety.

Work Details

The contract involves the provision of Emergency Medical Kits and Supplies as specified in Attachment C – Specifications/Drawings/Plans. Bidders are required to complete all necessary documentation as outlined in the Bidder Checklist, including the Request for Quotation (RFQ/RFB) and any applicable addenda. Specific items and quantities are expected to be included in the RFQ Workbook/Market Basket referenced in Attachment B.

The geographic location for performance or delivery of products is implied to be within the City of Los Angeles.

Bidder Requirements

Bidders must complete all forms listed in the Bidder Checklist, including: Request for Quotation Signature Page, Bidder Instructions and General (Attachment A), and any applicable forms related to local ordinances such as Living Wage and Worker Retention Ordinance (LWO/SCWRO) Forms. Additionally, bidders must comply with Business Inclusion Program (BIP) requirements and submit documentation related to potential MBE/WBE/SBE/EBE/DVBE/OBE bids or quotes received.
