Background

The New York State Office of Mental Health (OMH) is seeking to procure Optometry Services for the Creedmoor Psychiatric Center, located at 79-25 Winchester Blvd, Queens Village, NY 11427. The contract aims to provide essential optometric care to consumers at the facility, ensuring compliance with relevant regulations and standards.

Work Details

The contractor must provide a qualified New York State licensed optometrist who will perform on-site optometry services. Responsibilities include: acting as an optometry consultant; complying with Creedmoor Psychiatric Center MSO By-Laws; assessing and evaluating consumer conditions to develop treatment plans; reviewing treatment goals and medical histories; performing and interpreting tests; administering appropriate treatments; preparing documentation for medical records; consulting with treatment teams; submitting monthly statistical information regarding services provided; and being supervised by the Medical Director of the center. The facility will provide necessary supplies and materials, while the contractor must ensure all staff complete required training on privacy and security provisions of HIPAA.

Period of Performance

The anticipated contract term is from September 1, 2026, to August 31, 2027.

Bidder Requirements

The contractor must be a Service-Disabled Veteran-Owned Business (SDVOB) or meet other specified certifications. They are required to have a current New York State license and comply with all federal, state, and local regulations governing optometry services. Background checks against the Staff Exclusion List (SEL) and Statewide Child Abuse Registry List (SCR) are mandatory for individuals providing services.
