Health Risk Screening Tool - I/DD

Background

The Maryland Department of Health (MDH), specifically the Office of Developmental Disabilities Administration (DDA), is seeking to improve its current systems for assessing health and welfare risks among participants with intellectual and developmental disabilities. The agency aims to consolidate multiple existing web-based tools into a single screening tool that can be utilized by nonclinical staff to effectively determine participant needs, thereby enhancing service delivery and ensuring better health outcomes.

Work Details

The contract involves the development of a web-based screening tool designed to identify and assess health and welfare risks, functional status, behavior, and physiological factors for approximately 25,000 participants in the DDA waiver program. The tool will support non-clinical staff in conducting screenings and will be accessible to case managers, nurses, and participant teams for effective person-centered planning. Key functionalities include: - Integration with existing systems such as Long Term Services and Supports (LTSS) Maryland. - Identification of risks before crises occur. - Support for data collection, program planning, and resource management. - Utilization of results by various stakeholders including case managers and administrative teams.
