Background The Department of Veteran Affairs, Network Contracting Office (NCO) 16, is issuing a sole-source purchase order to Engineered Care, INC. under the authority of 41 U.S.C 253(c)(1). The goal of this contract is to provide a software solution that enhances the patient discharge process for veterans at the G.V. (Sonny) Montgomery Veterans Healthcare System in Jackson, MS. Work Details The contractor will deliver a software solution that includes: - Creation of After Hospital Care Plans with health literacy appropriate patient discharge instructions. - Report functionality for continual quality improvement and performance tracking. - An administrative interface for user management and application configuration. - Integration with VistA software for evidence-based discharge instructions. - Development of a patient-friendly discharge instruction booklet that reduces unnecessary readmissions and educates veterans about their care and VA programs. - Installation, configuration, and customization of the AHCP software at GVSM VAMC, including user account creation and branding requirements. - Provision of periodic performance reporting during installation and deployment, with real-time access for Jackson VAMC to verify compliance. Period of Performance 30 days after receipt of order (ARO), with options for additional years. Place of Performance G.V.
