Summary

The 355th Contracting Squadron is in the market research phase of a potential requirement and is looking for interested and capable sources for the following: Human Performance Optimization (HPO) The contractor shall provide all personnel, equipment, tools, materials, transportation, labor, supervision, management and quality control necessary, except as specified in performance work statement (PWS) paragraph 3.0, Government Furnished Property, Equipment, and Services, to perform Human Performance Optimization (HPO) Program Services, as defined in this performance work statement (PWS). The contractor shall perform injury prevention and mitigation, rehabilitation services, and HPO services to personnel assigned to the Davis-Monthan AFB (DMAFB), located in Tucson, Arizona. The contractor shall furnish consultation and reports, like the range of services found in a commercial facility, as well as services specialized to meet the needs of assigned military personnel. Performance shall be in accordance with the professional standards of the Accreditation Association for Ambulatory Health Care (AAAHC), and as defined in the PWS. The contractor shall hire, manage, and schedule positions meeting the six (6) following services: 1) Athletic Trainer (Level I and II)

2) Certified Strength and Conditioning Specialist

3) Performance Dietitian as required. The contract will include a requirement for a designation of a primary and alternate Contract Manager. Each position shall be provided at DMAFB for the assigned personnel as requested by organizational needs. Positions will be staffed as needed, no guarantee that all positions will be funded. Bidder Requirements Bidders must have the capability to provide qualified personnel for the specified roles including Athletic Trainers and Certified Strength and Conditioning Specialists.
