Background

The State of Utah - Division of Facilities Construction and Management (DFCM) is seeking renewal documents from Stage I Prequalified Contractors in various disciplines including Electrical, General, Mechanical, Paving, Roofing, and Site/Civil Hazmat. This request is aimed at contractors whose prequalification status is nearing expiration to ensure they remain eligible for future bidding on Stage II prequalified solicitations.

Work Details

Contractors must submit renewal documents to maintain their eligibility for Stage II prequalification. The required documents include: 1. Company Name and Prequalified Discipline(s). 2. Name and contact details of the Owner or Authorized Representative with legal signature authority. 3. Certification of statements regarding the contractor's status, including a declaration that they are not debarred or suspended by any government agency. 4. Period of Performance

Renewal documents must be submitted within 60 days prior to the expiration of the contractor's current prequalification status.

The contract will be performed in the State of Utah.

Bidder Requirements

Contractors must be currently prequalified through the Stage I process and have legal signature authority for document submission. They must certify their compliance with all statements required for renewal and provide explanations if unable to certify.
