They are required to submit a Statement of Ownership Disclosure form, a Non-Collusion Affidavit, and a Contractor Questionnaire/Certification along with their proposal. Additionally, bidders must provide proof of business registration in New Jersey and ensure that all employees who will interact with students have undergone criminal history background checks.
