The contract seeks a provider to deliver comprehensive services to chronically homeless individuals who are prioritized for housing and referred by the San Luis Obispo County Coordinated Entry System (CES). The primary work includes: - Housing placement for eligible clients - Case management tailored to individual needs - Individualized supportive services to ensure clients are able to sustain long-term, permanent housing Clients will be identified using the VI-SPDAT 3.0 or another Housing Needs Assessment tool approved by the San Luis Obispo County Homeless Services Oversight Council. The Housing Now program, formerly known as The 50Now Program, has been contracted by the San Luis Obispo County Department of Social Services since 2014. Its mission has always been to provide comprehensive services by leveraging community resources and addressing the individualized needs of chronically homeless individuals to secure and maintain permanent housing. Over time, findings and recommendations presented to the San Luis Obispo County Board of Supervisors highlighted an increased need for housing options, especially for specialized populations such as seniors, youth, and those experiencing domestic violence. In response, the program expanded its services and was renamed Housing Now. The program is recognized as a critical component for stabilizing chronically homeless individuals and reducing public service costs associated with homelessness.
Services must be provided to clients referred through the CES, using the agreed-upon assessment tools. Providers must offer housing placement, case management, and individualized supportive services, with an emphasis on sustaining permanent housing. The program must also be responsive to the needs of specialized populations identified by the County.
