Relative Caregiver Program Case Management Services

Background

The State of Tennessee, through its Central Procurement Office, is issuing this Request for Qualifications (RFQ #35910-14926) to procure qualified vendor(s) to provide Relative Caregiver Program (RCP) Case Management Services. The goal of this procurement is to stabilize relative caregiver households, promote child safety and well-being, and support permanency by delivering comprehensive support services to caregivers who have taken custody of children outside of formal foster care. This initiative aims to prevent disruptions in placements that could lead to adverse outcomes for children and increased likelihood of state custody entry.

Work Details

The selected vendor(s) will provide the following services under the Relative Caregiver Program: 1. Case management services including assessments, service planning, and caregiver support. 2. Administration of the RCP Stipend for eligible families. 3. Coordination with the Department of Children’s Services (DCS) and community partners. 4. Compliance with statutory and policy requirements. 5. Development of Individualized Service Plans tailored to each caregiver's needs. 6. Ongoing case management including home visits and monitoring caregiver participation. 7. Documentation and reporting as required by DCS, including monthly, quarterly, and annual reports.

Period of Performance

The contract will be performed over a five-year period from FY27 to FY31, with specific projected numbers of children served per fiscal year outlined in the solicitation.

Place of Performance

Services will be provided across various counties in Tennessee as specified in Table 1 of the RFQ, which includes groups such as Bedford, Coffee, Franklin, Davidson, Shelby, among others.

Bidder Requirements

Respondents must demonstrate qualifications including but not limited to: - A statement confirming no conflicts of interest. - Evidence of business standing with financial institutions. - Experience in providing similar services. - Compliance with all applicable laws and regulations including nondiscrimination policies. Additionally, respondents must hold necessary licenses and insurance before contract signing.
