Background The State of Delaware, through its Department of Health and Social Services, is seeking to engage a Vendor to provide support services as part of the Rural Health Transformation Program. The goal of this contract is to enhance health services in rural areas, ensuring that the needs of these communities are adequately met through professional and technical assistance. Work Details The Vendor will provide a range of professional services as specified in the appendices to the agreement. These services include but are not limited to: conducting assessments, developing strategies for health improvement, providing training and technical assistance, and facilitating stakeholder engagement. The Vendor is responsible for ensuring compliance with all applicable federal and state laws, including HIPAA regulations regarding Protected Health Information (PHI). They must also appoint a Project Manager to oversee the execution of these services and ensure quality control throughout the project duration. Period of Performance The initial term of the contract will commence on the Start Date and will continue for a specified number of years, with options for renewal based on mutual agreement between Delaware and the Vendor. Place of Performance The services will be performed primarily within the State of Delaware, particularly focusing on rural areas as designated by the Department of Health and Social Services. Bidder Requirements Vendors must be properly licensed to conduct business in Delaware. They are required to maintain compliance with all applicable laws and regulations, including obtaining necessary insurance coverage such as Worker’s Compensation, General Liability, and any other relevant policies as outlined in the contract. Additionally, Vendors must have an active registration with SAM (System for Award Management) if they are receiving federal funds.
